You are right, it's the right hand external door that opens in the VC.
The reason why it doesn't open in the external view is that Manfred got unfixable mesh distortions when he tried cutting the door out in the external model. So he left it closed.

The crew door only opens when you are in the VC. It does not open in external view.

This is not unusual, especially with turboprop aircraft. When FS loads the plane, it puts the engines through some power cycle (for some reason unknown to me), and this often shoves the plane forward. Putting the brakes on is about the only cure I know, unless you want to change the flight dynamics ...

As I remember the gates at the Heathrow 1961 are not assigned (other than some BEA and BOAC gates), so it should work fine for you. If you need some larger gates for heavies, just use AFCAD to increase a few to Gate Heavy.
