As I have a FLT licence I am thinking "There but for the grace of God go I". It is so easily done :-#

yes everybody makes mistakes , but I think this one started before he started the engine .. hot head maybe?

notice the exhaust gas as it come out of the tail pipe .. .. heavy and hard
