I don't recall seeing this posted before and I found it interesting since I have made similar mistakes in FS on ILS approaches, even with a functional radio altimeter. This is a video re-enactment of a 1/2009 crash.

Just google'd the details of that particular incident as I don't recall it.

Only nine fatalities including the three Pilots. *-)
