Actually, the problem is that everyone "didn't" do the right thing apparently. The moment the 2nd Row starter raised the red flag (visible in the first video) and Thom opened his cockpit, the start was supposed to be held until they could get him clear or the racers behind notified. The start wasn't waved off and the 3rd row pilots weren't notified so they took off, even though they weren't supposed to. The mitigating issue in the whole thing is that this all occurred in only a few seconds, so the time to realize and react was very slim before an accident occurred since the engine problems happened so close to the start.
Personally, having watched this, I think that IF1 will make a modification to either how planes are lined up on the start (staggering alternating rows or going in pairs instead of trios), or they will modify the start procedure that if a plane "declares" before the flag is dropped, they completely wave off the start and clear the problem aircraft.
To me, the bigger issue came that same day when one of the Gold Bi-Plane racers landed fast, bounced several times, and then ran into the winning aircraft on the runway because he didn't go-around like he should have. That impact, while wing-to-wing, was at a much higher speed and had a much higher chance of doing serious harm to the pilots of both planes.
