I am speculating , because I do not know what control system the 154 has, in particular for the rudders , but a conceivable explanation is if there was a yaw damper failure . Tonks will tell us, but IIRC the VC10 can show fairly spectacular Dutch Roll characteristics if the Damper is inoperative

It appears to me that he lost aileron control and was trying to land with rudder/elevator/engines only.
