October 2006 - ZS-OMF - Jetstream 4100

October 2006 - ZS-OMF Jetstream 4100 (SA Airlink) operated by Mocambique Expresso left Durban for Maputo.(3crew, 17pax)

*Climbing through FL120 the prop ice indicators warn about icing conditions.
*Captain cancels warning and switches on de-icing on both propellors and continues climb toward FL210.
*At FL180 both engines flame out.
*Captain switches on engine anti-icing on both engines.
*Left hand engine relit automatically after 6 seconds. Right hand engine did not relight.

*Soon after this the aircraft entered an uncontrolled roll to the right.
*Over the next 75 seconds the aircraft completed two full rolls, the first in a steep nose-down attitude (-70deg).
*The captain pulled the arcraft up at 4000ft where it pulled +5g.
*The second roll began at 45deg nose up and ended at 30deg nose down.
*Between the two rolls, the aircraft reached over 400 knots IAS and lost 13000 feet in 45 seconds.
*During the descent, over speed situations were evident on both props and engines.

*At FL050 the pilot regained control but the aircraft continued descending rapidly.
*The captain managed to restart the right hand engine manually at below 1000ft AGL.
*Here-after he initiated a climb and returned to Durban.

Damage to aircraft - R/H wing /fuselage - cracks
R/H wing/fuselage fairing - cracks
Forward wing Nacelle Fillet - Disbonded along wing and deformed. (corner flapped up)

On examining the R/H engine it was found that the Negative Torque Sensing (NTS) system was faulty and did not work following the flameout.

Bloody hell! Could be the first Xmas that somebody truly and sincerely meant thank you for their gift of Calvin Klein underpants.

by all accounts of the damage , it did crash
